Providence/Meadows Consent to Treat

As part of OISA Registration, Mt. Hood Meadows requires all riders to complete a Providence Consent to Treat Form.

Medical History Form

The medical history form is due back to the team before the first dryland practice. The information contained in the form will be used only in the event a rider is injured on the mountain during practice and/or competition. Email completed forms to wlhs.oisaliaison@gmail.com.
